What Is a Maintenance Dose for Weight Loss Injections?

Medically reviewed by Dr Saira Bano, BMBS, BMedSci, MRCGP, PGCert MedEd, SCOPE-Certified Obesity Specialist

Most of the conversation around weight loss injections focuses on the first few months: starting treatment, stepping up the dose and managing side effects. Far less is said about what happens once the weight has come off. Yet for many people, this is the phase that decides whether their results last.

That phase is built around a maintenance dose. This guide explains what a maintenance dose is, how it differs from the dose you lost weight on, how clinicians decide what is right for you, and how long you can stay on it.

What is a maintenance dose?

A maintenance dose is the dose you stay on once you have reached your goal weight, or a stable weight you and your clinician are happy with. The aim changes at this point. During weight loss, the dose is gradually increased to help you lose weight. During maintenance, the aim is to hold that weight steady on a dose you tolerate well, for as long as treatment is right for you.

Both Mounjaro (tirzepatide) and Wegovy (semaglutide) are started at a low dose and increased step by step, usually every four weeks, to give your body time to adjust. [1][2] The product information for each medicine describes the doses that can be used for ongoing treatment once that escalation is complete. For Mounjaro, the recommended maintenance doses are 5 mg, 10 mg or 15 mg once weekly. [1] For Wegovy, the usual maintenance dose is 2.4 mg once weekly, with a lower maintenance dose an option for people who do not tolerate it. [2]

Which of these is right for you is a clinical decision, based on how you have responded, how well you tolerate the medicine, and what you are trying to achieve next.

Is the maintenance dose the same as the dose I lost weight on?

Not necessarily. Many people simply continue on the dose that got them to their goal, and that is often a sensible choice. But higher is not always better. Some people reach their goal on a mid-range dose and never need to go further. Others find that once their weight has stabilised, a lower dose is enough to keep appetite in check.

The guiding principle we use is the lowest dose that keeps your weight stable and your appetite manageable. A lower dose can mean fewer side effects and lower ongoing cost, which both make long-term treatment easier to stick with.

What matters is that any change is planned and monitored, rather than made on your own. Dropping too far too quickly can let appetite return faster than expected, and it is much easier to adjust a plan early than to recover lost ground later.

How do clinicians choose your maintenance dose?

At SwiftDoctor, your maintenance dose is agreed at a review with one of our clinicians rather than set automatically. The things we look at include:

  • Your weight trend. Whether your weight has been stable over recent weeks, or is still drifting down or creeping up.
  • Your appetite and food noise. How easy it feels to eat well, and whether hunger is starting to return between doses.
  • Side effects. Whether nausea, reflux, constipation or tiredness are making your current dose hard to live with. Our guide to side effects of weight loss injections covers the common ones.
  • Your health more widely. Blood pressure, blood sugar, other conditions and any medicines you take.
  • Your goals and circumstances. Whether you want to stay on treatment long term, reduce gradually, or plan towards stopping, and what is affordable for you.

From there, we will agree a dose and a simple plan: how often we will check in, what weight range you are aiming to stay within, and what we will do if your weight starts to move outside it.

How long can you stay on a maintenance dose?

There is no fixed time limit in the product information for either medicine, and clinical trials show why ongoing treatment matters for many people. In the SURMOUNT-4 trial, people who continued tirzepatide after their initial weight loss kept losing weight, while those switched to a placebo regained a large part of what they had lost. [3] A similar pattern was seen with semaglutide in the STEP 4 trial. [4]

This reflects the fact that obesity is a long-term condition, similar to high blood pressure or high cholesterol, where stopping treatment often means the benefit fades over time. Some people therefore choose to stay on a maintenance dose for the long term, with regular reviews. Others prefer to reduce gradually and come off. Both are valid choices, as long as they are planned.

It is worth knowing that on the NHS, access to Wegovy through specialist weight management services is currently limited to a maximum of two years. [5] Privately, at SwiftDoctor, we support long-term maintenance where it remains clinically appropriate.

Can you lower your maintenance dose over time?

Yes, and for some people this is the natural next step once their weight has been stable for a while. Stepping down is done one dose level at a time, with a check-in after each change to see how your appetite and weight respond. If things stay steady, you may be able to step down again. If your weight starts to rise beyond the range you agreed with your clinician, you can move back up.

You may see suggestions online about stretching the time between injections or splitting doses. Please do not change how you take your medicine without speaking to a clinician first. These approaches are not how the medicines are licensed or designed to be used, and they make it harder to know what dose you are actually getting.

What else helps during the maintenance phase?

Medication is only one part of keeping weight off. The habits you build during treatment matter just as much, and arguably more once appetite starts to return. The most important are:

  • Protein at every meal, to help you feel full and protect muscle. Our guide on protein and strength training explains how much to aim for.
  • Strength training two or more times a week, as recommended in UK activity guidelines. [6]
  • Weighing yourself regularly, so small changes are spotted early, while they are still easy to act on.
  • Regular check-ins with your clinician, so your dose keeps matching your needs.

How SwiftDoctor supports you on maintenance

We specialise in helping patients keep weight off once they have reached their goal. Our weight loss maintenance plans are led by our GP team and include a personalised maintenance dose, regular clinical reviews, a planned approach if you want to step down or stop, and the option to restart within 12 months if you need to.

If you have reached your goal with another provider, you can also move your maintenance care to us. Book a maintenance consultation and our clinicians will talk you through your options.

Frequently asked questions

What is the maintenance dose of Mounjaro?

The product information lists 5 mg, 10 mg and 15 mg once weekly as recommended maintenance doses. [1] The right one for you depends on your response, side effects and goals, and should be agreed with a clinician.

What is the maintenance dose of Wegovy?

The usual maintenance dose of Wegovy injections is 2.4 mg once weekly, with a lower maintenance dose an option if 2.4 mg is not tolerated. [2] Your clinician will advise which is right for you.

Do I need to stay on the highest dose to keep the weight off?

No. Many people maintain their weight on a lower or mid-range dose. The aim is the lowest dose that keeps your weight stable and your appetite manageable.

Will I regain weight if I lower my dose?

Some people notice their appetite increase a little after a dose reduction, but this does not always lead to weight gain, especially with good habits in place. That is why we step down gradually and check in after each change.

How often should my maintenance dose be reviewed?

Regularly, and whenever your weight, appetite, side effects or health change. Your clinician will agree a review schedule with you as part of your maintenance plan.
